Xylitol and Heart Health: New Study Raises Concerns
In a groundbreaking study presented at ESC Congress 2026, researchers have found a potential link between high blood levels of the artificial sweetener xylitol and an increased risk of major adverse cardiovascular events. The study, which is the largest of its kind, analyzed data from a long-term observation of the general population, revealing that individuals with the highest xylitol levels had a significantly higher risk of experiencing death, myocardial infarction, or stroke compared to those with the lowest levels.
Specifically, over a 6-year follow-up period, the risk of a major adverse cardiovascular event was 57% higher in those with the highest xylitol levels. Even over a longer 30-year follow-up, the risk remained elevated at 18%. These findings are particularly concerning given the increasing use of sugar substitutes like xylitol in processed foods marketed as healthy alternatives over the past decade.
However, it is crucial to note that this was an observational study, meaning it can only show an association, not prove causation. As with all observational studies, causality is difficult to infer, but the researchers emphasize that this work mandates further investigation into this societally important topic. The study also included a separate experiment where participants ingested a xylitol-sweetened drink, and researchers observed a significant increase in every measure of platelet clotting ability immediately after ingestion, unlike with glucose-sweetened drinks.
These findings add to the growing body of evidence suggesting that artificial sweeteners may have unintended health consequences. While more research is needed to fully understand the relationship, this study serves as a critical reminder to approach sugar substitutes with caution. As the use of xylitol and similar sweeteners continues to rise, it is essential for both consumers and healthcare professionals to stay informed about potential risks. The authors of the study stress that further studies are mandated to clarify the long-term cardiovascular effects of xylitol, and until then, moderation may be key.
